Medicaid is a public health care program funded by the state and federal government. Medicaid provides health care coverage to certain individuals with limited income. Residents of Ohio may receive health care coverage at little or no cost if they meet specific income and eligibility requirements.
Effective January 1, 2014, Ohio began offering the Medicaid Extension program under the expanded guidelines due to the Affordable Care Act (ACA). Under these new guidelines, low income adults without children may now be eligible for pubic health care.
You may apply for Medicaid assistance online (fastest) here at or by filling out the Request for Cash, Food and Medical Assistance form and submitting it to Marion County Job & Family Services, 363 W. Fairground St., Marion, OH 43302, fax to (740) 387-2175, or email to MarionCounty@JFS.Ohio.gov.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (833) 620-1071, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to:
